New Patient Information

What to Bring to the Appointment

Current insurance cards including all health insurance and prescription insurance cards, drivere's license, previous health records, a list of all medications you are currently taking including the strength and how often you take them, a list of all your doctors and their telephone numbers, your completed Living Will and Medical Power of Attorney if you have these, and be prepared to pay a co-payment and or a deductible if needed.

What you may be asked:

First time visit (Medical History Questionnaire): As a new patient, you will be asked to share your past medical background. Please bring all pertinent information with you. Please print and complete the following forms:

Every Visit: We recommend dressing in comfortable clothes and footwear. For your appointments. Bring a sweater or throw, since it can be chilly in the clinic and treatment room. You and your doctor have a lot of information to share in a short amount of time. Please come with a written list of questions you would like to discuss. You may also find it helpful to bring a small notebook for keeping notes.

Emergency/After Hours: If you have a life-threatening emergency, call 911. Otherwise, please call the office and have the answering service contact the physician on call.
